Digital Waiver for CWTAA Students

Please complete this state-mandated online digital waiver. It is mandatory to attend in-person classes.

In consideration for my attendance and participation in martial arts training at City Wing Tsun Inc., and/or any class locations affiliated with City Wing Tsun Inc., I, on behalf of myself and any minor child/children for whom I have the capacity to hereby acknowledge and agree to the following:

1) I acknowledge the inherent risks in martial arts training and also the risks associated with the novel coronavirus ("COVID-19") or otherwise. I hereby agree to assume all risks and responsibilities and willingly choose to be on the premises or City Wing Tsun Inc. and participate in martial arts activities therein.

2) I fully assume all risk of illness or death related to COVID-19 or otherwise arising from my being on the premises and hereby release, waive, discharge and covenant not to sue, on behalf of myself and my minor children, City Wing Tsun Inc., its owner(s), officers, agents, employees, guest instructors and fellow students from any liability resulting from personal injury or loss of or damage to personal belongings arising from such risk as described herein.

(a) I did not have any COVID-19 symptoms in the past 14 days

(b) I was not tested positive for COVID-19 in the past 14 days

4) I give City Wing Tsun Inc. the right to take photographs of me training martial arts on the premises and I also grant them permission to use my image or likeness in photographs, video or other electronic means for advertisining or promotional purposes such as, but not limited to, their websites and social media accounts.

3) I also hereby declare that by being on the premises of City Wing Tsun Inc.,
